Join the TAKEheart Learning Community

Every year, over one million Americans qualify for cardiac rehabilitation (CR); however, only 20% of them participate. The AHA Center for Health Innovation is recruiting hospitals and healthcare systems for a new initiative to increase utilization of cardiac rehabilitation (CR). Hospitals and healthcare systems are invited to join the TAKEheart Learning Community now at no cost here to exchange information with a network of peers about a broad set of evidence-based strategies and to overcome mutually identified barriers to increase CR referral, enrollment, and retention. AHRQ’s TAKEheart initiative uses Training, Awareness, Knowledge, and Engagement to overcome those barriers and increase CR participation.

Hospitals are also welcome to apply to become a Partner Hospital for Cohort 2, which begins in late 2020. Partner Hospitals will participate in a 12-month program focused on increasing CR by implementing automatic referral with care coordination. Learn more about the free resources offered by TAKEheart or to join the TAKEheart Learning Community here.

Population and community health leaders are coming together to look at community, community health, and health equity through one lens, sharing innovative strategies and new tools to tackle tough challenges – and boldly transform how communities and populations experience health and care. At the ACHI 2020 National Conference, we come together as a community of population health leaders to amplify our shared commitment and message through our conference theme: Connecting Care with Community to Advance Health Equity.

The conference will engage more than 700 participants in stimulating dialogue and activities through:

  • Over 40 breakout sessions focused on topics such as maternal health, health equity, behavioral health, and more
  • Two intensive workshops: a train-the-trainer workshop for AHA partnership toolkits, and a social determinant of health “how to” series of workshops and panel discussions
  • Three immersive community experiences, highlighting hospitals and community partner organizations in Cleveland neighborhoods
